Employment packet for new employees or newly paid employees.

Welcome to the City of Rye Recreation Department. We are excited to offer you a position in our department. Please complete all forms attached and submit. Please have all information and attachments readily available (Social Security Card (signed) and Photo ID) as they are required to be uploaded to complete the process. If you are interested in direct deposit, please notify your direct supervisor.

Please note:
  • Please submit your signed Social Security Card and Photo ID within 5-days of submitting your paperwork on-line if you were not prompted to upload them during the process.
  • W-4 form. If you are tax exempt – please WRITE EXEMPT in line 4C
  • You must complete EITHER the NYS IT2104 OR NYS IT2104E (exempt candidates only).
  • For applicants under 18, working papers are required and should be submitted in person within 1 week after submitting this employment packet. Working paper applications are available at your high school. Those under age 15 must submit a "Blue Card"; those 16-17 years old must submit a "Green Card".
  • Direct deposit is now REQUIRED for all staff. A seperate URL will be provided for you to submit information and a voided check or other bank documentation with account and routing information.
